Throughout our programs, we offer an integrated approach to case management and supportive services which gives our customers seamless access to SCAP's continuum of services.This approach ensures the best possible outcomes for those we serve!

Community Crisis Network (CCN) - SCAP Community Resource Navigators provide front-line help to stabilize individuals and families in crisis and prevent future crises by securing needed resources. CCN partners are: SCAP, City Mission, Schenectady Inner City Ministry, Catholic Charities of Schenectady County, and the YWCA of NENY.

Homeless Prevention and Court Advocacy -We keep tenants and landlords out of eviction court, keep people in their homes and prevent homelessness through advocacy, mediation between landlord and tenant, intervention with City Court evictions, and securing resources to help both the tenant and landlord.

Sojourn House - Transitional supportive housing for homeless women with children, and homeless pregnant women. This home is a safe, nurturing and stable environment where women can work toward achieving their goals. We emphasize finding and maintaining permanent housing, development of life and parenting skills, and career planning. Meals are provided and the house is staffed 24/7.

Permanent Housing Program - Serves homeless families where an adult family member suffers from a disability such as mental illness, substance abuse or AIDS/HIV. Families receive stable, permanent housing, while developing the skills needed to become self-reliant and focusing on career planning.

Supportive Housing Solutions - Assists chronically homeless individuals and families who have a history of substance abuse or mental health problems. Our case managers link customers to permanent housing, mental health and/or addiction treatment services, legal assistance, employment services, or other community resources.

Home Connections - Serves DSS referrals to stabilize and link homeless families who are temporarily sheltered in motels with permanent housing.

VITA Income Tax Preparation - SCAP coordinates county-wide income tax preparation services for low and moderate income individuals by appointment. Expert assistance is provided to ensure you receive standard deductions and the Earned Income Tax Credit. This service is provided free of charge.
